Herts Sport & Physical Activity Partnership (HSP) wanted to encourage the PCN network to be more physically active to support the Active Practice Charter.
- Engage at least 50% of PCNs in active movement to ensure broad participation.
- Provide an always-on, accessible movement initiative as part of the Active Practice Charter.
- Empower PCN colleagues to prioritise their physical health by integrating movement into their daily routines.
- Continually highlight the importance of movement to support mental and physical health issues such as MSK.
- Support PCN colleagues to move more with a view to raise awareness of the importance of prescribing physical activity.

To address these challenges, the Total Active Hub team designed and deployed the Active Connections Hub across PCN's within Hertfordshire:
- Launched the Active Connection Hub to inspire colleagues to move more together
- Launched with a movement campaign on Blue Monday (20th January) to counteract seasonal blues with positive momentum.
- Focused on simple & inclusive movement that encouraged just 20 minutes of daily physical activity
- Deployed a multi-layered digital tracking platform that enabled people to move more, whether inactive or currently active.
- Easy tracking of activity via smart devices, converted minutes into points, and allowed participants to compete in teams within their surgery, PCN and countrywide.

The Active Connections Hub has significantly increased colleague movement and team engagement.
- Over 320 PCN colleagues engaged in the pilot programme
- 96% of users are now more mindful of the importance of daily movement
- 71% of colleagues felt more energised as a results of the hub activity
- Colleagues completed 255-minutes of active movement each week (compared to CMO's guidelines of 150 minutes)
- Created a lunchtime spike of physical activity across the PCN network

The Active Connections Hub was created to provide one platform to promote and encourage primary care staff to be more physically active. This was achieved by implementing the following:
- Always-on Calendar: Monthly movement events catered to all fitness levels, ensuring consistent engagement.
- Flagship Events: Special events such as the 'Remove the Blues' added highlight moments to drive engagement.
- Gamification: Deployed leaderboard to enable surgeries and PCN's to compete in friendly competition.
- Movement for Good Initiatives: Employees were incentivised to participate in movement challenges, linking their physical activity to CSR goals.
- Monthly Webinars: Focused on topics to motivate non-movers to move more - for example: How to find time to exercise.
- Personalised Engagement: Delivered daily motivational nudges and the ability to record activity through various methods. Developed a community where users could upload photos of their activities.

The Active Connections Hub has created energy and engagement across PCNs. Here are some headline stats from the ReMove the Blues event:
- 5,305 activities completed
- 7,553 miles covered
- The equivalent of 208 days of non-stop movement
- Walking was the most popular activity – 55%
- 17% of activities tracked as a workout
